FOMA Mid-Year Seminar

September 12-14, 2008

The FOMA 2008 Mid-Year Seminar will be held September 12-14, 2008 at the Grand Hyatt Tampa Bay in Tampa, Florida. Included in the program will be the Florida mandatory hours of 1-A CME credits: Risk Management, Florida Laws and Rules, Prevention of Medical Errors, and Uses and Abuses of Controlled Substances. Also being offered will be 1 hr of HIV/AIDS and Domestic Violence. The Seminar will offer approximately 25 hours of 1-A CME credits.

Advance Registration
If you pre-register for the Seminar, your complete packet, including name tags, tickets, programs, etc. will be at the FOMA registration desk when you check in.  FOMA members receive a $25 discount if you register prior to August 12, 2008.

Cancellations
The FOMA must be notified of any cancellation by August 21, 2008, in order to provide a refund.  Cancellations received on or before this date will be charged a $50 administrative fee.  Cancellations received after August 28, 2008 will not be refunded.
